Proudly Sponsoring A $1,000 Cash Scholarship!

Dr. Vacek is an advocate for higher education and he believes in giving back to the community of Waverly, as well as the surrounding communities we serve. With that said, our office will be sponsoring a $1,000 cash scholarship, available to local high school seniors. You can find additional information about the application process below. If you know someone who is a deserving candidate, please let them know about our scholarship opportunity!

This scholarship is available to high school seniors graduating with the class of 2022. This scholarship application form and a portrait photo must be dropped off at our office or e-mailed to amberlydental1@gmail.com no later than February 1, 2022.

General Instructions to Applicant

• Please complete the application form above.
• Return a neatly typed application and a portrait photo to our office by the deadline due date (applications and photos can also be submitted to amberlydental1@gmail.com).
• Dr. Vacek and his team will review applications and ten finalists will be selected. Those finalists will be notified and the finalists’ photos in addition to a brief bio will be posted on our office Facebook page for voting. Voting will be open to the public from March 1, 2022, until April 1, 2022. (This gives your friends and family a chance to vote for you!)
